重要零部件材料证明

产品铭牌及生产许可证
在显著位置处必须固定铭牌和标明生产许可证标记和编号，铭牌内容至少包括制造厂名、产品名称、产品型号或标号、制造日期或编号。

基础应有施工记录、材质检验报告等资料，大型游艺机的基础应有经有关检验部门检验合格的证明。
