带管出院患者延续护理中的作用

【摘要】目的带管出院患者安全管理的效果。方法对肿瘤中心2011年1~12月平均每周
带管出院患者,实施健全置管档案、统一导管维护时间、分时段落实电话随访、特殊时间段无缝隙管理、讲解配合演
示的带管安全教育、建立和发展片区维护网络的干预措施。%降为0,导管感染并发症
在5/千导管日以下,导管按时维护执行率为95%以上,全年无投诉和纠纷。结论带管出院患者的主
动干预和延续服务,保证了带管患者安全,提升了专科护理质量。

【Abstract】 Objective To evaluate the effect of safety management by intravenous therapy team for patients with after
discharge. Methods The intravenous therapy team provided continuity care for about 300 patients with after discharge
per week in the cancer centre from January to December in 2011. The measures included establishing documents for
patients,unifying the maintenance time,providing telephone follow-up,seamless management during special periods,intensifying
safety education by explaining and demonstrating,developing work of maintenance service. Results The inci-
dence of catheter pulling out was decreased from % to zero,the rate of catheter-related infections was below 5 per 1000
catheter-days,the implementation rate of maintenance was above 95% ,the incidence of plains and disputes
