AUTHORITY: Title 5, Section 3012; Title 10, USC, . 9397
PRINCIPAL PURPOSES: To permit the Chain mand to maintain information and records vital to the soldier and necessary to the Chain mand in case of emergencies.
ROUTINE USES: To update information required by the military for daily purposes. Information furnished may be disclosed to DOD officials who need this information to perform their duties; the American Red Cross; and relatives. The SSN is used for positive identification.
DISCLOSURE: Voluntary. Disclosure of SSN is voluntary.
