(1) In filling the mendation form,in what capacity
推荐人和你的关系,一般填Professor、Teacher或者Advisor。

指的是拥有永久居留权,即绿卡。
(3) Temporary address & Permament address该怎样填
The temporary address填现在的住址,比如学校,但需注明何时终止。如果你不注明,通常I-20会寄到permanent address。或者在Permanent address处填写你现在宿舍的地址,在temporary address处填写你租用的信箱的地址。
(4) List the faculty you have contacted or expected to contact concerning your application for admission to XXX Univ.
如果你和某个教授联系过,可以填上教授的名字,不会有坏处。对于有些学校的一些系,这项是非常重要的,需要注意。
(5) Source of Funds
如果你申请全额奖学金,就不需要填写这一项,或者多少填一些,也可以添:content。
