Brief history 
n  This is a 45 y/o woman suffered from 
acute cholecystitis and laparoscopic 
cholecystectomy was planned. 
n  The known preop data included: 
ASA class II 
BW:45kg 
Hb:
Brief history 
n  Drops both in SpO2 and ETCO2 were 
noted when creation of 
pneumoperitoneum was attempted. 
n  The management include: resumption 
of trandelenburg position and pumping 
air out of abdomen 
n  Operation was resumed and the course 
was smooth hereafter.
Treatment of acute 
cholecystitis 
n  Medical: NPO, NG suction, IV fluid, 
analgesia, and antibiotics. 
n  Surgical: open (OC), laparoscopic (LC) 
and minimal invasive (MC) 
cholecystectomy
Comparison between LC, and 
OC 
n  1. No evidence of symptomatic e 
between LC, and OC. 
n  2. Weak evidence that mortality and 
complication rates were lower after LC 
than OC. 
n   evidence that biliary tract 
complication were mon in LC.
