【摘要】目的探讨脑电图对脑白质损伤(WMD)早产儿的预后评估价值。方法对确诊为WMD的46例早产儿行脑电图检查。结果 46例发生脑白质损伤的早产儿中PVE Ⅰ度21例,PVE Ⅱ度11例,PVE Ⅲ度14例。脑室旁白质一过性病变28例,脑室旁白质持续性病变18例。脑白质损伤不同程度的各组之间脑电图异常有显著性差异(H=,P<)。随诊脑白质损伤为一过性病变与持续性病变组脑电图异常有显著性差异(u=,P<)。结论脑电图能正确反映WMD病儿的脑功能状态,对WMD预后的评价具有重要意义。

[ABSTRACT]ObjectiveTo investigate the electroencephalogram (EEG) for evaluation of the prognosis of premature infants with white matter damage (WMD).MethodsEEG monitoring was performed in 46 premature infants with Ⅰwas found in 21 cases, PVE Ⅱin 11 and PVE Ⅲ in 14. Temporary WMD of periventricular region was noticed in 28, while persistent WMD in 18. Differences in EEG manifestations between temporary WMD and persistent WMD were significant (u=,P<).ConclusionEEG may exactly reflect the state of brain function, which is of important role in the assessment of the prognosis of premature infants with WMD.

随着产科和新生儿重症监护诊疗技术的提高,早产儿尤其是极低出生体质量儿以及超低出生体质量儿的存活率显著提高。随之而来的是早产儿脑损伤的问题越来越受到重视,约10%存活的早产儿以后出现痉挛性运动缺陷,有25%~50%早产儿以后主要表现为认知或行为缺陷或轻度运动障碍,高神经系统发病率主要与脑白质损伤(WMD),尤其是脑室周围白质软化(PVL)有关[1]。文献报道,在早产儿中PVL的总发生率约为8%~25%;在应用呼吸机的早产儿中,发生率高达35%~60%[2]。如何早期发现并正确判断其预后对减少早产儿脑损伤所致的伤残有重要意义。本文对46例确诊为WMD早产儿的脑电图进行回顾性分析,以探讨脑电图对脑白质损伤病儿诊断及预后评估的价值。
1 对象与方法

2002年3月~2005年5
