某三甲医院规模经济与科室规模扩张现状及影响因素研究
摘要
目的:通过对某综合性公立医院的投入要素和产出要素进行分析,并建立生产函数模型,深入探讨
医院内部规模经济发展状况。为提升医院的核心竞争力,有针对性的改进医院内部工作质量和效率,
促进医院的内部管理,提升内部监管和控制能力,提供理论依据。
方法:本研究根据样本医院卫生管理信息系统提供的数据,现场调查了样本医院及选取的具有代表
性的 5 个临床科室 2006 年至 2009 年的数据,采用描述性统计方法对样本公立医院和样本科室的规
模现状及医疗质量和效率等指标进行描述和对比分析;运用 Cobb-Douglas 生产函数对医院的投入产
出进行分析;运用 TOPSIS 法对样本医院和代表科室的综合效益进行分析。
结果: ,截至 2009 年全院床位数较 2006
年增长 29%,其中骨科和妇产科床位数增加最多。 年医院出院人次环比增长 24%,其中医保
患者占全院出院人次的 45%,环比增长 56%。,医院综合效益在 2008 年出
现大幅下降,但在 2009 年全院综合效益上升到历年最高水平。,
医院卫生技术人员和床位数对卫生服务量和收入呈正向作用,其中卫生技术人员对产出的影响更加
显著。
结论:1. 2009 年样本医院床位数量扩张后处于规模收益递增阶段。增加对卫生技术人员、资产和床
位数的投入有利于提高医院的产出量(服务量和医疗收入)。2. 综合效益不一定会随着床位数量的
扩张而增加。2009 年骨科、肿瘤科床位数达到历年最高,但因为展开床位使用率大幅下降而导致综
合效益指数历年最低。显示出骨科和肿瘤科在现阶段不宜继续扩张床位数。

Abstract
Objective: Through prehensive investment in public hospitals to analyze elements and
output elements, and the establishment of the production function model, in-depth study of
economies of scale within the hospital development. To enhance the petitiveness of
the hospital, targeted to improve the quality and efficiency within the hospital to promote the
hospital's internal management, improve the internal supervision and control capabilities,
provides a theoretical basis.
Method: The purpose of this study is to sample hospital hygiene management information
system to provide the data, on-site survey the hospital and selected representative five
