【摘要】目的近年来,国外流行病学研究发现糖尿病人群中甲状腺功能异常的发生率明显增高,是非糖尿病者的2-3倍。糖尿病与甲状腺功能减退症均是常见的内分泌代谢性疾病,二者并存并非少见,有时症状叠加互相影响,对于糖尿病合并甲状腺功能减退症患者应两病兼治。

【Abstract】 In recent year, the studies by foreign epidemiology show that the incidence of thyroid dysfunction in diabetic patients increases significantly, which is as twice or three times as in non-diabetic patients. Both diabetes mellitus and hypothyroidism mon endocrinology and metabolic diseases. The phenomena that the two diseases occur simultaneously are not mon. More, sometimes the symptoms of two diseases interact. So, it should treat both of the diseases simultaneously for the diabetic patients with hypothyroidism.

病例一:患者,女,72岁。因口渴、多饮12年,加重伴双下肢浮肿半个月入院。时症见:口渴多饮,尿频量多,双下肢浮肿、麻凉痛,乏力,畏寒,偶有胸闷气短,便秘,夜寐差,病来无恶心、呕吐、发热等症。既往有冠心病史30年,高血压病病史10余年(血压最高达180/100mmHG)。查:℃,P78次,R18次,BP130/70mmHG,,神清,面色少华,体型肥胖,语言欠流利。全身皮肤干而粗糙,头发稀少。颈软,甲状腺未扪及肿大。双肺呼吸音清,未闻及干湿罗音。心界不大,心率78次/min,律齐,各瓣膜区未闻及杂音。腹软,肝脾肋下未扪及。双下肢有凹陷性水肿,双足背动脉搏动弱。理化检查:×109/L,×1012/L,HGB122g/L;尿常规GLU1+;肾功Na139mmol/L,,,,UA221 umol/L;,,HDL-,LDL-;随机血糖13mmol/L;肝功能、胆红素测定均正常。,,,THGAB
