【摘要】目的评估16排螺旋CT双期三维血管造影(3D-CTA)显示脑膜瘤与邻近血管关系的能力。方法回顾性分析61病理证实的脑膜瘤术前双期3D-CTA影像资料,与手术结果进行对照,评估3D-CTA显示脑膜瘤与周围脑血管关系的能力。结果以手术为标准,本研究对42例与临近大动脉接触的脑膜瘤CTA判定为A型(肿瘤未侵蚀血管)28例,24例与术中发现一致(%),CTA判定为B型(肿瘤侵蚀血管)14例,12例与术中发现一致(%)。对28例靠近静脉窦的脑膜瘤,CTA判定有临近静脉窦侵犯的共18例,全部被手术证实;其中14例CTA判断其侵犯程度与手术一致。结论双期3D—CTA提供了脑膜瘤与邻近大血管、静脉窦及骨骼三维空间准确关系;可明确区分肿瘤周围的动静脉成分;有利于手术方案的制定及提高术中处理的可预测性。

[Abstract] Objective To explore the role of double-phase 3D 16-slice MDCT angiography(MDCTA) in the preoperative evaluation of intracranial vascular involvement induced by The preoperative double-phase 3D-CTA image data of 61 pathologically confirmed meningioma was analyzed retrospectively. The results pared with intra-operative findings. The ability of the relationship between meningioma and its surrounding vessels with 3D-CTA was evaluated. Results Regarded the surgical findings as standard, this study of 42 patients with artery close contact with the meningiomas; CTA revealed that 28 cases were type A(surrounding arteries remained intact), 24 of which were consistent with the surgical findings; 14 cases were type B (erosion of artery) 12 of which were consistent with the surgical findings.
