【摘要】目的探讨对慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者应用BiPAP辅助通气治疗时的观察和护理方法。方法将2003年4月~2007年5月我院ICU收治的COPD合并Ⅱ型呼吸衰竭患者63例随机分为两组,对照组32例,治疗组31例,在综合治疗基础上,对照组予中心管道鼻导管氧疗,治疗组予应用BiPAP呼吸机无创通气治疗,对两组生命体征、血氧饱和度、pH值、动脉血氧分压、动脉二氧化碳分压进行护理观察并对比。结果治疗组实施BiPAP呼吸机无创通气后4 h,呼吸困难减轻,呼吸频率及心率下降,经皮血氧饱和度及动脉血氧分压均明显升高,动脉二氧化碳分压下降,与对照组疗效比较,差异有统计学意义()。结论 BiPAP呼吸机治疗Ⅱ型呼吸衰竭有确切的疗效。对患者进行心理护理和保持呼吸道通畅是使用BiPAP呼吸机治疗的前提;各项生命体征、意识状态、血气指标是BiPAP呼吸机治疗时的观察重点。

[Abstract] Objective To investigate the observation and nursing methods of BiPAP ventilation on patients of chronic obstructive pulmonary(COPD)with type Ⅱ respiratory 60 patients of COPD with type Ⅱ respiratory failure,hospitalized in ICU from April 2003 to May 2007,were divided into 2 groups randomly:control group(n=32)and treated group(n=31).On the basis of prehensive treatment,the control group were treated with oxygen through nose tube,and the treated group were treated with sign,arterial blood oxygen saturation(SaO2),pH value,partial pressure of oxygen(PaO2),partial pressure of carbon dioxide(PaCO2)were In treated group after 4 h of BiPAP ventilation,the decrease of dyspnea,and heart rate,increase of SaO2 and PaO2,and reducing of PaCO2 were significantly different from BiPAP ventilation has a definite therapeu
