河北省基本医疗保险制度城乡一体化研究

摘要
河北省自 1999 年开展实施城镇职工基本医疗保险以来,先后建立了针对农村居民
的新型农村合作医疗和针对城镇非从业居民的城镇居民基本医疗保险。目前三项基本医
疗保险已经在制度上覆盖了全省居民,但由于三项基本医疗保险制度所依据政策措施不
同、经办管理机构分离、筹资和补偿标准不一致等现状,导致现行基本医疗保险制度呈
现二元分离的特性,这种城乡分离的医疗保险制度带有非常强烈的歧视性,与公平性原
则相违背,不利于基本医疗保险制度互助共济功能的发挥,不能有效地解决人民群众“看
病贵”的难题,针对这些问题,通过探索建立城乡一体化的基本医疗保险制度可以加快
促进基本医疗保障体系的建设,从而和公共卫生服务体系、基本医疗服务体系、药品供
应保障体系建设协同推进、共同发展,加快促进基本医疗卫生制度的建立和人人享有基
本医疗卫生服务目标的实现。
本文主要以三项基本医疗保险的制度措施和新医改的各项意见为指导,在充分了解
各项基本医疗保险制度运行现状的基础上,对河北省实现基本医疗保险制度城乡一体化
的必要性和可行性进行分析,总结出尽管我省进行基本医疗保险制度城乡一体化面临一
些发展困境,但实现基本医疗保险制度城乡一体化是我国医疗保障发展的最终目标。所
以结合我省发展实际提出了适合我省运行的基本医疗保险制度城乡一体化路径设计,最
后通过对内部政策进行调整和外部环境优化来充分保障基本医疗保险制度城乡一体化
的顺利实施。

Abstract
Since 1999 the implementation of Hebei province in the basic medical insurance for
urban workers, has established a since for rural residents of new rural cooperative medical and
aimed at the non-employed residents of urban resident basic medical insurance. At present
three basic medical insurance has covered in the province in the system, but because three
residents of the basic medical insurance system based on policies and measures that
organization of management institution of different, isolation, financing pensation
standard is not consistent with the status quo, lead to the basic medical insurance system for
the characteristics of present dual separation, the urban and rural separation system of medical
insurance with very strong discriminatory, and fairness principle, to the detriment of curbing
the basic medical insurance system help each other in all aid functions, cannot effectively
solve the masses expensive medical cost, aiming at these problems, the urban and rural
integration through exploration established the basic medical insurance system can speed up
to promote the construction of basic medical security system, thus and public health service
