丙基硫氧嘧啶的严重不良反响与药学监护点

摘要 目的：分析丙基硫氧嘧啶的不良反响发生规律，为临床安全使用丙基硫氧嘧啶提供参考。方法：以“propylthiouracil and side effects〞和“丙基硫氧嘧啶〞为检索词检索Medline和KI，将所查阅文献分析统计。结果：丙基硫氧嘧啶的严重不良反响报道有上升趋势，除之前被大家所熟知的血液系统和肝脏损害外，又有更为严重的不良反响，如抗中性粒细胞胞浆抗体〔antineutrophil cytoplasmic antibody ANCA〕阳性小血管炎被报道。但早期发现与时停药，根据患者病情给与相应治疗，患者转归较好。结论：提高大家对丙基硫氧嘧啶不良反响的认识有助于该药的安全使用。

Abstractobjective: To analyze the regularity of adverse reactions of propylthiouracil and to providereference for it’s clinical safety utilization. Methods:To retrievePubmed and KI respectively by means of keywords“side effects〞 and ( or ) “propylthiouracil〞, then analyze the literatures. Results: Except hematological system injury we are familiar with, more severe adverse reactions such as antineutrophil cytoplasmic autoantibody-positive vasculitis(ANCA) were reported, cases of which tended to ascend. If early detection and drug withdrawal promptly, correspondingtherapy was adopted according to the kind of adverse reactions, the patient would get a good oute. 丙基硫氧嘧啶是一个硫脲类衍生物，被广泛的用于治疗甲亢。尽管它相对安全，但自从1947年上市以来，已经有几类相对严重，威胁生命的不良反响被报道，如肝脏毒性、血液系统毒性、免疫系统毒性，尤其是抗中性粒细胞胞浆抗体〔antineutrophil cytoplasmic autoantibody ANCA〕阳性小血管炎，近年报道逐渐增加，引起大家的关注。我们就丙基硫氧嘧啶查阅了大量文献，并加以分析，发现一些不良反响的发生规律，现报告如下，希望能给医生和药师提供参考。
1、方法 以“propylthiouracil and “ side effects〞和“丙基硫氧嘧啶〞为关键词查阅Medline和中国期刊全文数据库KI〔1994-2008〕，共检索到英文文献532篇，中文文献98篇，逐篇阅读摘要，对其中完整的患者病历资料按照年龄、性别、国籍、服用剂量、合并用药、原患疾病、不良反
