of rural drinking water sources, protection of drinking water sources in rural areas by the end：从2003年8月5日《乡村医生从业管理条例》公布之日起，进入村医疗卫生机构从事预防保健和医疗服务的人员，必须具备执业医师资格或者执业助理医师资格。
第七条：乡村医生申请执业注册，应向村医疗卫生机构所在地县级人民政府卫生行政主管部门提交下列材料：
（一）乡村医生执业注册申请审核表；
（二）申请人身份证复印件；
（三）二寸免冠正面半身照片两张；
（四）县级以上地方人民政府卫生行政主管部门颁发的乡村医生证书；
（五）下列证书或证明之一医|学教育网搜集整理：
1、《乡村医生从业管理条例》颁布以前取得的省级以上人民政府教育行政部门认可的中等以上医学专业学校毕业证书；
2、省人民政府卫生行政主管部门颁发的《河北省乡村医生中专水平培训合格证书》或《医生（乡村）资格证书》；

3、由乡镇卫生院出具并由县级人民政府卫生行政主管部门审核认定的在村医疗卫生机构连续工作20年以上的证明；
4、按照本办法第五条规定接受培训、考试，取得的由省级人民政府卫生行政主管部门统一印制的乡村医生执业考试合格证书。
（六）村医疗卫生机构出具的拟聘用证明和《医疗机构执业许可证》（副本）；拟新建村医疗卫生机构的须提交村民委员会推荐证明和县级人民政府卫生行政主管部门拟批准设置的证明。
第八条：县级人民政府卫生行政主管部门应当自受理申请之日起15日内完成审核工作，对符合规定条件的，准予执业注册，发给由省人民政府卫生行政主
