Acknowledgments
This book was written over the span of several years. At times it
seemed that it would never pleted. Now that it is done, I feel
that there is much more that could have been said, read, and written
on this topic. Yet it does represent my thinking at this point in time
and will hopefully be of some use to others who are grappling with
