鉴于：
化学事故应急救援工作的目标是最大限度地减少化学事故造成的人员伤害、财产损失和环境破坏。《危险化学品安全管理条例》和《危险化学品登记管理办法》规定危险化学品生产企业必须向用户提供化学事故应急咨询服务，为危险化学品事故应急救援提供技术指导和必要的协助。企业提供24小时应急咨询服务（Ⅰ级应急服务），承担社会救援的义务，以便其产品在运输和使用过程中发生事故时，救援者能及时了解产品的成分、危险特性、应急措施等信息。
为了规范我国的化学事故应急咨询服务网络， 浙江联大漆业有限 公司（以下简称甲方）、浙江省安全生产科学研究院（以下简称为乙方）、国家安全生产监督管理总局化学品登记中心（以下简称丙方）三方经友好协商，在平等的基础上签订本咨询代理协议，共同遵守。
一．合作概况
三方的合作基于以下事实：
甲方是化学品的制造商或分销商，在中国境内生产、储存或销售危险化学品；
乙方为甲方所在（省、自治区或直辖市）的危险化学品登记管理机构。乙方负责组织该地区的危险化学品登记、登记材料的审核、本地区危险化学品登记管理数据库和动态统计分析系统的建立，并提供化学事故应急咨询服务；
丙方已按照国际模式在全国开通了24小时化学事故应急咨询电话专线0532-83889090，该电话已被国家安全生产监督管理总局指定为国家化学事故应急咨询电话。
二．服务内容
丙方同意甲方在产品的安全技术说明书和安全标签上标注国家化学事故应急咨询专线电话的号码0532-83889090。
丙方向甲方在中国大陆的产品用户提供Ⅰ级应急咨询服务。丙方提供Ⅰ级应急咨询服务所需的安全技术说明书（MSDS）及其它相关资料由甲方提供。
