【摘要】[目的]分析人工颈椎间盘置换术的近期疗效及其并发症并分析其原因。[方法]2006年6月~2007年12月,对60例患者的73个节段进行了Bryan人工颈椎间盘置换术,男33例,女27例;年龄33~54岁,。[结果]全部病例随访13个月~2年,所有患者术后症状明显改善,术中术后没有发生血管和神经损伤并发症,1例神经根型患者术后根性疼痛缓解不明显,可能与术中减压不彻底有关,患者拒绝再次手术,给予保守治疗效果不佳。脊髓型颈椎病患者术前JOA评分为(±)分,末次随访时为(±)分,与术前比较差异有显著性()。颈痛视觉评分VAS、上肢疼痛视觉评分、颈部功能异常评分指标(neck disability index,NDI)均与术前有显著性差异()。1例术后1年随访时发现假体周围异位骨化形成,2年随访时出现自发融合。[结论]人工颈椎间盘置换治疗颈椎间盘疾患可取得较好的临床疗效,但手术操作复杂,有其特有的并发症,应重视手术适应证的选择及严格手术操作规范。

Abstract: [Objective]To explore the cause of the plications of Bryan cervical disc arthroplasty and summarize the prevention experience. [Methods]From June 2006 to December 2007, 60 patients (33 males and27 females) with 73 levels underwent Bryan cervical disc placement. The average age was years( range,33-54 years).[Results]One plained of persistent arm pain because of plete pression and was treated by dehydration, but the pain was still present. The average period of follow- up was months, JOA scores were improved from ± points to ± points () .One case showed heterotopic ossification in 1 year and developed spontaneous fusion in 2 years after operation.
