【摘要】目的探讨葡萄胎中Mel-CAM与滋养细胞黏附、浸润之间的关系以及Mel-CAM标记中间型滋养细胞(IT)的增殖情况。方法采用双重免疫组化法,检测Mel-CAM标记IT的Ki-67表达情况。结果葡萄胎中Mel-CAM表达较正常绒毛明显增高,差异有统计学意义(P<);葡萄胎绒毛表面有Mel-CAM标记IT出现,而正常绒毛表面则无;葡萄胎组织中IT的Ki-67指数较正常绒毛明显增高,差异有统计学意义(P<)。结论葡萄胎绒毛表面有Mel-CAM表达,表现出双重分化方向,这可能与葡萄胎的发病机制有关;检测葡萄胎组织中IT的Ki-67指数对于预测葡萄胎的转归有潜在的价值。

ABSTRACT: Objective To explore the relationship between Mel-CAM and adherence and invasion of trophoblastic cells and proliferation of Mel-CAM defined intermediate trophoblast. Methods Double immunohistochemical staining technique was used to detect the expression of Ki-67 in Mel-CAM defined intermediate trophoblast in human villi during the first trimester and hydatidiform mole. Results In this study there was a significantly stronger expression of Mel-CAM in hydatidiform mole than that in first-trimester villi (P<). The expression of Mel-CAM in hydatidiform mole was observed on the surface of villi, but not in human villi. The Ki-67 index of hydatidiform mole was higher than that it of human villi with a significant difference (P<). Conclusion Mel-CAM might play an important role in the invasion of trophoblastic cells. The different differentiation type of villous trophoblast in hydatidiform mole and the first-trimester villi may be correlated with pathogenesis. 中间型滋养细胞(intermediate trophoblast, IT)是正常胎盘和妊娠滋养细胞疾病(gestational trophoblastic disease, GTD)中一类特殊的滋养细胞类型。随着研究的深入,逐渐认识到IT是一类独立的细胞类型,有自身特殊的免疫组织化学和生物学特征,并且IT的独立增生可以形成胎盘部位滋养细胞肿瘤(placental site trophoblastic tumor, PSTT)、超常胎盘部位(exaggerated placental site, EPS)、胎盘部位结节(placental site nodule, PSN)和上皮样滋养细胞肿瘤(epithelioid trophoblastic tumor, ETT)等多种良、恶性疾病。但是,由于IT的细胞形态学特征介于合体滋养细胞(syncytiotrophoblast, ST)和细胞滋养细胞(c
