【摘要】目的探讨子宫内膜腺癌组织中微血管密度(MVD)与临床病理因素和细胞增殖的关系。方法应用免疫组化EnviSion 法检测20例正常子宫内膜和70例子宫内膜腺癌组织中CD34和细胞增殖指标Ki67的表达情况。结果子宫内膜腺癌组织中MVD明显高于正常子宫内膜组织(t=,P<),MVD与病人年龄、病理分级无关(t=,F=,P>),而与手术病理分期、子宫肌层浸润深度及淋巴结转移均有关(F=,t=、,P<、);子宫内膜腺癌中Ki67阳性组MVD值显著高于Ki67阴性组,差异有统计学意义(t=,P<)。结论子宫内膜腺癌组织中MVD与肿瘤的生长、浸润和转移有密切关系,联合检测MVD和Ki67可作为一种衡量子宫内膜腺癌生物学行为和判断预后的指标。

[ABSTRACT] Objective To investigate the expression of CD34 of microvessel density (MVD) and the relationship among MVD, clinicopathological features and cell proliferation in patients with endometrial adenocarcinoma. Methods The expressions of CD34 and Ki67 in 20 cases of normal endometrium and 70 cases of endometrial adenocarcinoma were detected by immunohistochemical EnviSion method. Results MVD value in the endometrial adenocarcinoma was significantly higher than that in the normal endometrium (t=,P<). There was no relati
onship between the value of MVD and patient’s age,pathological grade (t=,F=,P>), but it was related to clinical stage, myometrial invasion and lymph node metastasis (F=;t=,;P<,). Expression of Ki67 was positively correlated with MVD (t=,P<). Conclusion MVD is associated with the growth, invasion and metastasis of endometrial adenocarcinoma. Combination detection of MVD and Ki67 can be used as a parameter to judge the biological behavior and prognosis of endometrial adenocarcinoma.

微血管密度(MVD)是反映恶性肿瘤侵袭和转移等生物学行为的一个重要指标。子宫内膜癌是严重影响中老年妇女生命健康的恶性肿瘤,其发病率有明显上升的趋势[1~4]。本研究以CD34为微血管染色指标,检测正常子宫内膜及子宫内膜腺癌组织血管生成的量,通过分析MVD与肿瘤组织的临床病理因素及细胞增殖之间的关系,探讨其临床意义。
1 材料和方法
材料
收集青岛大学医学院附属医院病理科1996年5月~2001年12月的子宫内膜腺癌手术标本70例,按WHO标准进行子宫内膜腺癌病理组织学分级,高分化癌23例,中分化癌32例,低分化癌15例;按国际妇产科联盟(FIGO,1988)手术
病理分期,Ⅰ期38例,Ⅱ期17例,Ⅲ期15例;≤1/2肌层浸润者30例,>1/2肌层浸润者40例;无淋巴结转移者42
