目的
慢性疼痛是血液透析患者常见的临床症状,但关于疼痛的病因和疼痛发生率目前
的研究甚少。本研究旨在探讨维持性血液透析患者的慢性疼痛发病率及慢性疼痛
的影响因素,从而对血液透析患者慢性疼痛的治疗予以指导。
方法
本研究入选 80 例在深圳市南山医院血液透析中心进行维持性血液透析至少 6 个
月以上的终末期肾脏病(ESRD)患者。使用简明疼痛量表(BPI)评估疼痛。对
于慢性疼痛的定义为疼痛至少持续 3 个月。搜集患者的年龄、性别、体重指数、
血液透析时间、原发病、血液生化指标。分析这些指标和慢性疼痛之间的关系。
结果
在我们选入的 80 例患者中,平均年龄是 46±14 岁;平均透析时间是 ±
个月。39 例(%)患者存在慢性疼痛,其中 16 例(20%)为中重度疼痛。肌
肉骨骼痛、神经性疼痛是疼痛最主要的表现形式。对于有无疼痛这两组,其各数
据行 t 检验显示两组的透析时间、血钙、钙磷乘积、甲状旁腺激素有统计学差异
(P<)。而年龄、性别、体重指数、血红蛋白、血白蛋白、血磷、C 反应蛋
白无统计学差异(P>)透析时间、血钙、血钙磷乘积、甲状旁腺激素(iPTH)
与疼痛的发生存在正相关,相关系数分别是 ,,,。
结论
血钙、钙磷乘积、甲状旁腺激素、透析时间和长期血液透析患者慢性疼痛密切相
关。

The analysis of relative influencing factors of chronic
pain in maintenance hemodialysis patients

Abstract
Objective
Chronic pain is mon clinical symptom of hemodialysis patients,wheras the cause
and the incidence of pain in current research is quite rare. This study aimed to
investigate the cause and the influencing factors of hemodialysis patients with chronic
pain, and give guidance to the treatment of chronic pain in maintenance hemodialysis
patients.
Methods
Using the Brief Pain Inventory (BPI) to assess pain, this study selected 80 patients who
had at least 6 months of MHD(maintenance hemodialysis ) in the hemodialysis center of
Nanshan hospital of Shenzhen. For chronic pain is defined as pain lasting at least 3
months, we have collected each patient's age, gender, body mass index, blood dialysis
duration, primary disease and blood biochemical parameters. And then use a
statistical method to analyze the relationship between these indicators and chronic pain.
