步行训练对脊髓损伤大鼠运动功能恢复的影响研究

目的明确步行训练对脊髓损伤大鼠运动功能恢复的促进作用,探讨其机制。
方法 20 只 3 月龄雌性 SD 大鼠随机分为对照组(A 组)和实验组(B 组),每组各 10
只。采用 NYU (New York University)法制作大鼠 T10 脊髓损伤模型。实验组在术后 1
周进行步行训练,共训练 8 周;对照组不接受干预。造模后每周进行
BBB(Basso-Beattie-Bresnahan)评分和斜板试验评分。术后第 9 周进行苏木精-伊红
(HE)染色、5-羟色胺(5-HT)和多巴胺-β羟化酶(DBH)免疫组化检测。
结果 BBB 评分:实验组较对照组训练第 3、8 周评分具有显著性差异(P<),训
练第 4-7 周评分具有非常显著性差异(P<);斜板试验评分:实验组较对照组训
练第 4、5、8 周评分具有显著性差异(P<),训练第 6、7 周评分具有非常显著性
差异(P<)。实验组 BBB 评分与斜板试验评分成正相关。光镜下对照组伤区仍可
见大量液化区,脊髓大部分呈噬洞样改变,周围环绕密集胶质纤维细胞;实验组伤区
液化区、空洞及胶质纤维细胞明显减少。免疫组化法观察:横切片 5-HT 和 DBH 阳性
神经元主要在脊髓灰质前角集中分布,实验组较对照组数量多且密集;纵切片脊髓损
伤尾侧 5-HT 和 NA 能阳性神经纤维实验组较对照组表达数量多且密集。
结论步行训练可促进脊髓挫伤大鼠后肢运动功能的改善,其机制与脊髓损伤部位尾
侧 5-HT 和 NA 能阳性神经纤维的再生及脊髓前角神经元对 5-HT 和 DBH 的表达增强有
关。

ABSTRACT
Objectives: To explore the effect of treadmill training on hindlimbs functional recovery
for rats with plete spinal cord injury(SCI) and its’ underlying mechanism.
Materials and Method 20 three-month-old female SD rats were randomly divided into
control group (A group) and experimental group (B group), 10 rats in each
cord contused model was made at thoracic 10 segments with NYU impactor device. The
experimental group received treadmill training 1 week treadmill training
last for 8 weeks. The control group did not receive hindlimbs functional
recovery was evaluated by the Basso-Beattie-Bresnahan(BBB)scales and incline plane test.
After training for 8 weeks, the expression of 5-HT and NA was detected by immunohisto-
chemistry staining.
Results BBB score was significantly higher in group B than in group A since 3 weeks
postinjury. The score of incline plane test was significantly higher in group B than in group
A since 4 weeks postinjury. The correlation coefficient is a positive correlation between the
