山西省城镇居民基本医疗保险制度研究

摘要

2007 年 7 月 10 日,国务院发布《国务院关于开展城镇居民基本医疗保险试
点的指导意见》,紧接着在 7 月 23 日,国务院《全国城镇居民基本医疗保险试
点工作会议》在京召开,温家宝总理和吴仪副总理出席了会议并作了重要指示,
启动了以 79 个城市为审批试点单位的城镇居民基本医疗保险试点,该政策计划
到 2010 年覆盖全国,届时它将与城镇职工基本医疗保险、新型农村合作医疗一
道构成我国医疗保障制度的“三驾马车”,医保制度将覆盖全国 13 亿人口,真
正实现“全民医保’。在全国范围内,山西省的城镇居民基本医疗保险工作走在
了前列,截止到 2008 年底,全省共有 152 万人参加城镇居民基本医疗保险,城
镇居民基本医疗保险费总收入 38249 万元,总支出 14703 万元。制度实施取得
了巨大成绩的同时,也暴露出很多问题值得我们探讨。
本文首先回顾了医疗保险相关理论,接着介绍了山西省城镇居民基本医疗
保险制度实施情况,从制度建设、城镇居民参保以及基金收支情况等方面初步
分析了山西省城镇居民基本医疗保险制度的运行情况,以山西省医疗保险服务
中心的资料和数据为依托,全面研究分析城镇居民基本医疗保险的制度建设情
况、城镇居民参保情况、参保居民的受益情况、基金筹集情况、门诊和住院费
用补助情况、经办定点机构建设情况以及定点医疗机构的建设管理情况,分析
总结出城镇居民基本医疗保险制度运行过程中出现的问题,最后针对这些问题,
从完善制度设计、强化政府部门职责、加强经办机构建设、加强定点医疗机构
建设管理、完善城镇居民基本医疗保险管理体制几个方面提出相应的政策建议。

Abstract

On 10th of July, 2007, the State Council issued the Opinions on Handing, Management and
Service of the Basic Medical Insurance of Resident in Cities and Towns, and then launched the 79
cities pilot work of basic medicial insurance for residents in cities and towns much better than the
other regions of the nation. In the end of 2008, there are 152 ten thousand people involved this
insurance , there is 38249 ten thousand dollars earned and 14703 ten thousand dollars spended . The
implementing of insurance system in Shan Xi acquires much achievement. But on the other side,
there are still a lot of problems exposed and needed to solve.
This paper introduces the implementing situation of Shan Xi basic medical insurance for
residents in cities and towns. It analyses the system’s operating situation from the
system-establishing, insurance for resident in cities and towns and funds’ e and expense. By
collecting and processing data, the paper studies and analyses the situation of system for consultant
for basic medical insurance and the management of named medical institutions, then summarize
