一、申办《保健食品经营企业卫生许可证》条件程序
（一）适应范围：揭阳市区（含大南山侨区、普侨区，下同）范围内申请经营保健食品的单位。
（二）审批依据：
1、《中华人民共和国食品卫生法》第二十七条；
2、《保健食品管理办法》；
3、《广东省食品卫生许可证发放管理办法》
（三）许可条件：
1、在本市区范围内从事保健食品经营的企业；
2、申报企业具备如下条件：
（1）符合《中华人民共和国食品卫生法》第八条的卫生要求；
（2）销售场所远离粪场、垃圾堆等污染物；
（3）销售场所内具备采光、通风、排烟、防蝇、防鼠、防尘、防虫、污水污物处理等卫生设施；
（4）从业人员经培训、健康检查合格；
（5）具有与经营规模相适应的经营及仓储面积；商品存放离墙隔地，分类存放；
（6）制订卫生管理制度。
3、申报资料合法、完整和规范。

（四）申报材料：
1、《保健食品经营企业卫生许可证》申请表；
2、法定代表人或经营负责人的资格证明(身份证复印件)；
3、经营场所和仓库总平面图，包括摆放、存放区划图；在超市等其他商业企业内设立零售保健食品店的，必须具有独立的区域；
4、提供所经营保健食品的批准文书和生产卫生许可证复印件；
5、保健食品质量保证体系的情况（包括卫生管理机构、人员及培训体检记录、质量管理规章制度等）；
6、许可机关需要提供的其他资料。
经营企业需提交以上资料（一式二份，所附的资料均须采用A4纸打印，图纸需标明面积及尺寸，并逐页加盖公
