字体及字号要求
题目为黑体，四号，居中；正文与题目之间空一行，首行缩进2字符，字体为宋体，字号为五号，单倍行距。
质量要求
本表由学生个人申报，经团支部民主评议后，将电子文稿和纸质文稿各一份上报院系素质拓展学分评定管理中心集中保管备案，由院系团委书记经审核无误后，进行院系级别认证工作。本着“谁组织、谁认证、谁负责”的原则，所上传材料必须是原创，不得抄袭。校级抽查不合格率超过15%的，整个院系不能予以认证，全体退回，并发函通报至院系团委；不合格率在15%以内的，限期一周进行整改，整改合格方可予以认证，不合格不予以认证。所有抽查结果将通过网站和电子屏进行公布。
3. 学分认证
学生积极参加暑假社会实践活动，认真落实并上交心得报告、提供认证照片等辅证材料，要求学生不得抄袭，200---1000字即可，由团支部认真评定，按五级分制给出“优”、“良”、“中”、“及”、“差”，并上报院系团委审核，由院系团委初步认证。其中 “优”比例原则上不超过10%，“良”或“中”，所占比例分别不超过30%；学生在寒暑假社会实践活动中表现优秀，获得校级及校级以上先进个人称号，其相应成绩为“优”；获得院系级先进个人称号，其相应成绩为“良”。所占比例总体上不超过50%，由院系初步认证，校团委最终审核。凡未经审核通过的学生个人，均不
