一、眼科医院(三级)---80张以上，不设一、二级眼科医院
二、骨科医院
三级---300张以上，二级---100张以上，不设一级
三、妇产医院
二级---50--200张，三级---200张以上，不设一级
四、耳鼻喉医院
1、二级耳鼻咽喉诊台及诊椅１５至２０台，住院床位总数５０至１００张。
2、三级耳鼻喉医院耳鼻咽喉诊台及诊椅３０台以上，住院床位总数７０?１５０张。
3、不设一级耳鼻喉医院
 
用A4纸、宋体小4号字打印，英文为12号字，资料按以下顺序排列,并装订成册。凡提交的材料均需加盖单位骑缝公章或法定代表人印章(签字)。
 
〔一〕拟设医疗机构所在地的市、州卫生局初审同意设置的意见、当地?医疗机构设置规划?、当地医疗机构设置公示情况；
〔二〕?医疗机构设置申请书?一式三份；
〔三〕设置可行性研究报告；
可行性研究报告包括以下内容：
〔1〕申请单位名称、根本情况以及申请人姓名、年龄、专业履历、身份证号码；
〔2〕所在地区的人口、经济和社会开展等概况；
〔3〕所在地区人群健康状况和疾病流行以及有关疾病患病率；
〔4〕所在地区医疗资源分布情况以及医疗效劳需求分析；
〔5〕拟设医疗机构的名称、选址、功能、任务、效劳半径；
〔6〕拟设医疗机构的效劳方式、时间、诊疗科目和床位编制；
