前 言
实施国家基本公共卫生服务项目是促进基本公共卫生服务逐步均等化的重要内容，也是我国公共卫生制度建设的重要组成部分。国家基本公共卫生服务项目自2009年启动以来，在城乡基层医疗卫生机构得到了普遍开展，取得了一定的成效。2011年，人均基本公共卫生服务经费补助标准由每年15元提高至25元。为进一步标准国家基本公共卫生服务项目管理，卫生部颁发了《国家基本公共卫生服务标准〔2011年版〕》，我省组织专家，结合我省实际，对服务标准内容进行了修订和完善，形成了《安徽省基本公共卫生服务标准〔2011年版〕》〔以下简称《标准》〕。
《标准》包括11项内容，即：城乡居民健康档案管理、健康教育、0～6岁儿童健康管理、孕产妇健康管理、老年人健康管理、预防接种、传染病及突发公共卫生事件报告和处理、卫生监督协管、高血压患者健康管理、2型糖尿病患者健康管理、重性精神疾病患者管理。在各项服务标准中，分别对国家基本公共卫生服务项目的服务对象、内容、流程、要求、考核指标及服务记录表等作出了规定。《标准》中针对个体服务的相关服务记录表单应纳入居民健康档案统一管理，考核指标标准由各地根据本地实际情况自行确定。
《标准》是乡镇卫生院、村卫生室和社区卫生服务中心〔站〕等城乡基层医疗卫生机构为居民免费提供基本公共卫生服务的参考依据，也可作为各级卫生行政部门开展基本公共卫生服务绩效考核的依据。《标准》所列基本公共卫生服务项目主要由乡镇卫生院和社区卫生服务中心负责组织实施，村卫生室、社区卫生服务站分别接受乡镇卫生院和社区卫生服务中心的业务管理，并合理承担基本公共卫生服务任务。城乡基层医疗卫生机构开展国家基本公共卫生服务应接受当地疾病预防控制、妇幼保健、卫生监督、精神卫生等专业公共卫生机构的业务指导。其他医疗卫生机构提供基本公共卫生服务可参照本《标准》执行。
鉴于国家基本公共卫生服务项目将随着经济社会发展、公共卫生服务需要变化和财政承受能力提高等因素不断调整，省卫生厅将根据实际情况适时对《标准》进行修订。在本标准颁发前印制的表格，假设无实质性差异的可以继续使用，但对缺失或不一致的内容要采取切实可行的措施补救，以到达要求。
