办理程序
（一）建筑工程
申请人持符合审查要求的材料向市政务中心建设分中心并联审批窗口提出申请。市政务中心建设分中心并联审批窗口将申请材料传递到第3并联审批窗口。第3并联审批窗口收到资料后，对建设单位提供材料齐全、符合法定形式的工程当场予以备案。
（二）市政工程
建设单位持相关质量文件（资料）向第3并联审批窗口提出申请，对建设单位提供材料齐全、符合法定形式的工程当场予以备案，并发给《建设工程质量监督书》一式三份。
申请材料
（一）建筑工程1、《成都市建筑工程和市政基础设施工程质量监督备案表（建筑工程）》一式三份并签章；2、《成都市建筑工程设计文件审查批准书》、《成都市房屋建筑工程施工图设计文件审查合格书》（查验原件，收复印件）。3、建设单位确定的工程质量检测见证取样送检人员授权书一份。注：凡在工程建设过程中增加了装饰、网架、钢结构、幕墙工
