（一）编委、人社等部门有关本单位机构成立的有效批文和单位基本账户开立文件。
（二）国家技术监督局制发的《组织机构代码证》、人社局制发的《事业单位法人登记证书》。
（三）主管部门对下属财务独立核算单位同意开户的证明。
（四）省级以上（含省级）政府或财政部门批准需设立财政资金专户管理的有效文件。
（五）其他相应证明材料。
第七条专户的变更
专户发生下列变更事项，必须在预算国库股进行备案。
（一）预算单位变更名称，但不改变开户银行及账号。
（二）预算单位主要负责人或法定代表、地址及其他开户资料变更。
（三）因开户银行原因变更银行账号，但不改变开户银行。
（四）其他按规定不需报经财政部门审批的变更事项。
第八条专户的撤销
（一）专户因特殊需要变更开户银行，应将原账户撤销，按本办法的规定重新办理开户，并将原账户的资金余额（包括存款利息）如数转入新开账户。

（二）对属于财政资金专户原批准开户文件已废止或政策执行到期，应将原专户撤销，对仍有结余资金的，应按规定将结余资金转出后及时办理销户手续。
（三）按规定开设的专户，在开立后一年内没有发生资金往来业务，该账户应作撤销处理并按本办法规定办理备案手续。
