螺旋CT诊断钝性肠管和肠系膜损伤的临床价值

[摘要] 目的探讨螺旋CT对急性钝性大小肠和肠系膜损伤的诊断价值。材料与方法 2004年2月至2008年5月,急性腹部钝性损伤螺旋CT扫描300例; CT诊断大小肠或/和肠系膜损伤18例,其中16例剖腹手术,2例临床随访;CT表现与手术探查对照分析。结果肠壁增厚的CT敏感性、特异性和准确性分别为75%、63%、63%;肠壁破裂的CT敏感性、特异性和准确性分别为13%、33%、100%;肠系膜血肿的CT敏感性、特异性及准确性分别为38%、86%、71%;肠系膜血管中断的的CT敏感性、特异性及准确性分别为40%、100%、100%;系膜束带征CT敏感性、特异性及准确性分别为83%、56%、56%;气腹或肠壁或系膜间积气敏感性、特异性及准确性、分别为75%、63%、50%等。结论综合分析肠和肠系膜损伤的直接和间接CT征象,可以较准确地评估肠和肠系膜损伤的程度及损伤部位;螺旋CT扫描可为临床治疗方案的决策提供重要的参考信息。

[Abstact] Objective To evaluate the value of spinal CT in the diagnosis of bowel and/or mesenteric injuries in blunt abdominal and Methods The CT manifestation and clinical material of eighteen patients with suspected bowel and/or mesenteric injuries from 300 blunt abdominal trauma patients were analyzed from February 2004 to May currently performed all helical CT examinations in patients suspected of having blunt abdominal injuries. Intraoperative findings(16 cases)and clinical observation(2 cases) pared puted tomographic(CT). RESULTS The CT sign of bowel wall thickening had sensitivity of 75% ,specificity of 63% and accuracy 63% ,the CT sign of bowel wall discontinuity had sensitivity of 13% ,specificity of 33% and accuracy 100%.
