比较研究

摘要
目的:通过体外离体牙实验,比较三种根管封闭剂在根管充填后根尖和冠方微渗漏
情况,评价其根管封闭性的优劣,并分析其可能原因,为临床选择更理想的根管封闭剂
提供一定的实验依据。方法:收集新鲜拔除的单根管恒前牙 60 颗,使用手用镍钛锉,
采用冠根向深入法进行根管预备。将预备后的离体牙随机分为三个实验组,每组 20 颗
牙齿,A 组:采用 Cortisomol 封闭剂+冷牙胶侧方加压充填技术,B 组:采用 AHplus
封闭剂+牙胶尖侧方加压充填技术,C 组:采用常温流动牙胶+单根主尖充填技术。根管
充填后,所有标本置于恒温箱使材料硬固,随后浸入印度墨水染色,硝酸脱钙、乙醇梯
度脱水、置于水杨酸甲酯溶液中透明处理。在体视显微镜下观察透明标本上根尖和冠方
染料渗入距离,用游标卡尺测量染料渗入深度。结果:对实验数据用 统计软
件包进行单因素方差分析,三组间两两比较采用 SNK 检验。(1)Cortisomol 组、AHplus
组和 Gatta-Flow 三组根尖部的染料渗入长度差异有统计学意义(P<),Cortisomol
组与 AHplus 组间的差异有统计学意义(P<),Cortisomol 组和 Gatta-Flow 组间的差
异有统计学意义(P<),AHplus 组与 Gatta-Flow 组间的差异无统计学意义(P>)。
(2)三组根管冠部的微渗漏长度差异有统计学意义(P <0. 01),Cortisomol 组与 AHplus
组和 Gatta-Flow 组间的差异有统计学意义(P<),AHplus 组与 Gatta-Flow 组间的差
异无统计学意义(P>)。结论:Gatta-Flow 常温流动牙胶充填系统能更佳封闭根管
根尖端和冠部,可有效防止微生物侵入导致再次感染,并操作时间短,可减少患者的痛
苦。在临床上建议推广应用。

Abstract
Objective: By the extracted teeth in vitro study, pare the sealing ability of
Gatta-Flow using single cone technique with that of AHplus and Cortisomol sealer when used
with laterally condensed gutta-percha obturation technique, and to provide the important
theoretic and experimental basis for the choice of perfect root canal sealer in clinic.
Materials and Methods:60 single rooted human anterior teeth were cleaned and shaped
using hand use NiTi instruments with a crown-down technique, after this ,all teeth randomly
divided into three experimental groups(n=20). Group A was filled with lateral condensation
gutta-percha+Cortisomol sealer. Group B was filled with lateral condensation
gutta-percha+AHplus sealer. Group C was filled with Gutta-Flow+s
