【摘要】目的判定白细胞去除在体外循环(CPB)中的作用。方法 20例接受心脏手术的患者被随机分为对照组和白细胞去除组(LD组)。在体外循环开始前,开始后10、30 min, CPB 停止、停止后2 h采集血标本作β2-integrin和L-selectin测定。术前、术后第1天测血浆肌钙蛋白(troponin I)。计算术前、术后氧合指数。结果 2组CD11a表达无明显差异(P=)。对照组CPB期间中性粒细胞CD11b表达高于LD组(P=)。对照组CD11c在CPB开始时就升高(P=),但与LD组比较差异无统计学意义(P=)。LD组L-selectin表达低于对照组(P=)。术后LD组血浆troponin I 低于对照组(P= )。结论 CPB期间白细胞去除是一种有效、安全易行的减少组织损伤、保护肺功能的方法。

Abstract: Objective To determine the efficacy and safety of leukocyte depletion in cardiopulmonary bypass (CPB). Methods 20 patients undergoing heart operation were randomly allocated to the leukocyte-depletion group (LD group) and the control group. Blood samples were collected at 5 time points: before stereotomy, 10 and 30 min of CPB, termination of CPB and 2 hours after CPB. The β2-integrins and L-selectin were determined. Blood samples were also collected before operation and l day postoperatively to determine troponin I. Oxygen index and mediastinal drainage were recorded. Results There was no significant change in CD11a expression level in either group (P=).The expression level of CD11b on neutrophils during CPB in the control group was significantly higher than in the LD group (P=).
