【摘要】目的探讨螺旋CT动态增强扫描在孤立性肺结节鉴别诊断中的价值。材料和方法分析24例经手术病理证实的肺内孤立性结节的CT动态增强扫描资料,包括肺癌18例和肺结核球6例。测量各结节CT动态增强扫描中不同时间点(0s、30s、70s、120s和180s)相同层面感兴趣区的CT值,描绘并比较两种结节的CT动态增强时间-密度曲线。结果肺癌结节CT增强扫描后各时间点平均CT值较增强前增高在30HU以上,而肺结核强化CT平均值增高在10HU以下。两种结节的CT动态增强扫描的时间-密度曲线形态不同。结论: 螺旋CT动态增强扫描有助于肺癌和肺结核结节的鉴别诊断。

[Abstract] Objective To evaluate the value of dynamic contrast-enhanced spiral CT in the differential diagnosis of solitary pulmonary nodule. Methods The data of dynamic contrast-enhanced CT scan was analyzed from 24 cases with pathologically diagnosed pulmonary nodule including 18 casea with lung cancer and 6 cases with pulmonary tuberculosis. CT values of the nodule at different delay time after injecting contrast medium were measured and the time-density curve of two different pulmonary nodules was described pared. Results The increasing mean CT value of lung cancer nodule between pre- and post-contrast scanning was higher than 30 Hu while that of pulmonary tuberculosis was lower than 10Hu. The shape of two time-density curves of dynamic enhanced CT was different. Conclusion Dynamic contrast-enhanced spiral CT is helpful to the differential diagnosis of solitary pulmonary nodule.
