研究背景与目的通过对国内外双向转诊理论研究和实践研究的把握,以及区域卫
生信息化实践活动规律的概况,本研究旨在构建出基于医院与社区的双向转诊健康信
息资源平台。具体研究目的有:解读新医改对双向转诊健康信息资源平台新要求;总
结出适用于平台的电子健康档案的管理路线,进而得出健康信息资源平台的集成架构
和应用架构;规范双向转诊流程,明确双向转诊健康信息资源平台的核心功能和社会
支持系统,保障双向转诊健康信息资源平台的顺利运行。
研究方法采用文献计量学方法,本研究收集国内外双向转诊和区域卫生信息化建
设相关资料,并对主要研究观点进行归纳和比较分析;通过关键知情人访谈、现场典
型调查和需求调研与分析,了解并分析目前双向转诊流程的问题以及双向转诊健康信
息资源平台的现状和需求;通过专题小组讨论,对卫生行政管理部门有关人员、医务
部门负责人、医院有关人员以及社区卫生服务机构人员的组织相关专题小组讨论,了
解建设双向转诊健康信息资源平台会存在的障碍,探讨行之有效的解决办法,并体现
到双向转诊健康信息资源平台中。
研究内容本研究首先重点把握国内外双向转诊存在的问题、对策、双向转诊流程
的规范以及区域卫生信息化建设实践的一般规律,进而研究电子健康档案在平台上的
有效使用和管理,以期通过电子健康档案来盘活整个平台;然后在分析平台上各方用
户需求基础上,重点进行双向转诊健康信息资源平台集成架构和应用架构设计;最后
关注双向转诊健康信息资源平台核心功能的实现,以及平台的社会支持系统建设,保
障本研究的应用价值。
研究结论研究显示:在区域卫生信息化进程中,能够用技术解决的问题都不是最
终的问题,即建立医院与社区卫生机构双向转诊健康信息资源平台,技术已经不是最
大障碍,关键在于设计并完善规范的双向转诊流程,合理发挥电子健康档案的核心贯
通作用,建立良好的社会支持系统,通过合理有效的管理来保障平台相关健康信息有
序流通和共享,进而推动双向转诊核心功能的实现。

ABSTRACT
Research background and purpose:
The paper started with cognition of the two-way referral in theory and practice at home and
abroad, as well as a summarization of the laws of regional health informatization, with the
respect to build the two-way referral of health information resource platform between the general
hospitals and munity units. The study aimed to unscramble the new requirements for
the construction of the two-way referral of the health information resource platform under the
new medical reforms, evolve the administrative procedures for the electronic health record, then
gain the integrated framework and the application architecture of the platform, and standardize
the referral procedures, finally clear and definite its core functions and social support system
to security the platform running smoothly.
