摘要
目的:探讨目前城乡并行的社区卫生服务和新型农村合作医疗制度科学、合理、
全面、有效对接与融合的可行性。方法:采取专项调查法,重点对自治区 10 个地级
城市和 8 个县市社区卫生服务机构和新农合制度实施情况进行调研,并查阅国家和
自治区制定的有关指导文件以及相关书籍、文章、年鉴以及自治区第三次国家卫生
服务调查结果,采取定量调查法和描述性研究,了解两项制度运行的基本情况和实
施效果;采用系统分析研究方法,对比分析城乡卫生制度的优劣,讨论已经具备的
基础条件和存在问题,最后提出制度衔接的重点和路径。结果:调查与分析表明,
社区卫生服务和新型农村合作医疗制度是新一轮医药卫生体制改革的突破口和试验
田,先期开展的试点和推广工作均取得显著成效,具有令人乐观的发展前景。两项
制度具有各自的比较优势,在定性定位、职责任务、发展方向和发展目标等方面具
有一致性,前期试点和推广为城乡卫生工作的一体化发展奠定了良好基础,已经具
备衔接的可行性。结论:社区卫生服务和新型农村合作医疗制度可以通过服务体系
整体科学规划、服务机构的标准化设置、建立统一完善的城乡基本医疗保障制度、
长效的财政投入机制和规范的资金管理制度、改革人员编制核定和人事管理制度,
充分发挥中医民族医药作用等方面来实现城乡基础卫生制度的有机衔接和统一。
