五、几点说明
⒈ ABO疑难血型判定提示：
⑴正、反定型不一致；
⑵与先前血型鉴定结果不一致；
⑶弱凝集、混合凝集或其它情况难以准确判定结果；
⑷与ABO同型血液交叉配血试验不相合；
⑸不符合一般遗传规律。
2. RhD抗原阴性判定及处理原则：
RhD抗原初筛试验阴性者；RhD抗原结果难以判定或/和先前鉴定不一致，均暂按RhD阴性血型处理。
⒊ 特殊情况紧急抢救输注血小板的建议：
⑴首选与受血者ABO/RhD血型同型血小板输注；
⑵在紧急抢救患者生命时，发现患者血型难以判断或血小板供应短缺情况下，可以选择不同血型的单采血小板输注。

⑶输注不同血型的单采血小板前，要向病人及家属进行风险告知并签署知情同意书。例如：供者血浆中的血型抗体引起急性溶血反应的可能；血小板输注无效的可能；RhD阴性患者输注RhD阳性血小板后有可能产生抗-D抗体，特别对育龄期妇女，可能发生流产、死胎、新生儿溶血病（女童患者成年后风险同上）等。
⑷输注不同血型的单采血小板，应选择抗-A、抗-B抗体效价≤64的供者，儿童应尽量减少血小板中的血浆量，以防止发生溶血性输血反应。
