    〔四〕实施时间
    本方案从2022年9月30日起实施。
二、考评组织和监督管理
1、医院成立医务人员医德医风考评工作领导小组，各科室也要成立医德医风考评小组，小组长由科主任担任，副组长由护士长担任，组织实施本科室医务人员的医德医风考评工作，并向医院医德医风考评工作领导小组上报考评结果。
2、医院医德医风考评办公室负责医德医风考评的日常管理、组织协调与实施，确保考评工作顺利进行。
3、各职能科室要根据要求，加强对医务人员的病历、处方、用药和检查情况的日常检查和问卷调查、患者反映、投诉举报、表
扬奖励等相关资料的收集，为年终医德医风考评提供准确、真实依据。凡科室不按照本方法对医务人员进行医德医风考评，或

有以下情形之一的，对该科室责任人予以通报批评。
⑴不履行考评职责或者未按规定履行职责的：
⑵在考评工作中有弄虚作假、徇私舞弊行为的：
⑶在考评过程中显失公平的；
⑷经院医德医风考评办公室监督检查，考评程序和考评结果评
定等存在问题，责令整改后，经检查仍不合格的。
4、工作要求
