摘要:
目的探讨老年患者压疮发生的原因,从不同层面进行立体分析,并提出整改应对措施。通过院内不良事件报告系统,收集全院护理人员在护理住院患者压疮中存在的问题,运用鱼骨图进行分析归纳。得出压疮的原因有病人内在疾病因素、病人营养不良、外在环境、安全监管不到位及护理人员安全意识淡薄。应用鱼骨图对住院患者压疮的原因进行分析,条理,直观,协助管理者从最直接的小刺入手进行“削刺”,逐步解决问题。

Abstract:
Objective To study the elderly patients with pressure ulcers occurred, three-dimensional analysis of the different levels, and proposed corrective measures to cope. Hospital adverse event reporting system to collect hospital-wide nursing care hospitalized patients with pressure ulcers, the use of the fishbone diagram, analyzed and summarized. Come to pressure sores because patients with intrinsic disease factors, patients with malnutrition, external environment, security and poor oversight and weak nurses' safety awareness. Application of fish bone diagram to analyze the reasons for hospitalized patients with pressure ulcers, coherent, intuitive, help managers to start from the most direct stab "cutting the thorns”, and gradually solve the problem.

鱼骨图是一种发现问题“根本原因”的方法,它也可以称之为“lshikawa”或者“因果图”。压疮是老年卧床患者人群中常见易发的护理问题,发生压疮不仅给患者增加痛苦,影响疾病康复,还增加了医疗费用,延长了住院时间。据统计[1],老年患者压疮的发生率为10%~25%,%,70岁以上患者发生率则达到70%[2].因此,对可能发生压疮的住院老年患者进行危险因素评估,并针对评估结果给予相应的护理措施,可以有效预防压疮的发生。本院根据老年住院患者安全隐患特点,实施鱼骨图分析管理,加强前馈控制,做好压疮的各项防范措施,收到了良好效果。
