Note that the Pulsatility Index (PI) of this LIMA-LAD graft was or less
than the mended PI threshold (PI>5) for reexamining the graft. The
low mean flow ( ml/min) indicated that the graft warranted attention.
The systolic dominant profile corroborated this quantitative observation.
After revision, flow increased sixfold to 60 ml/min and the curve exhibited a
classic profile now indicating a patent graft.
Defined as the difference between maximum and minimum flows divided by mean flow,
Pulsatility Index (PI) is bined measure of low flow and pulsatility.
A high PI (>5) bination with low mean graft flow does confirm that there is a
problem with a graft and it should be reexamined. However, a low PI, (as in the example
above) does not rule out a problem with the graft. Possibly caused by a severe stenosis at the
proximal anastomosis, a surgeon can easily replicate the condition by creating a near-total
occlusion in the graft upstream from the flow probe. PI, therefore, should not be used as a
