下肢运动功能的影响
【摘要】目的探讨胫前肌肌电生物反馈联合减重步行训练(BWSTT)对脑卒中偏瘫患者足下垂的疗效,并用表面肌电图进行定量分析。方法脑卒中患者72例采用析因设计方法随机分为常规组、减重组(BWSTT组)、肌电生物反馈组(简称肌电组)、肌电生物反馈联合BWSTT组(简称联合组)4组,每组18例,根据分组给予肌电生物反馈或(和)减重步行训练,并于治疗开始前一天及治疗6周后第二天立即用常规临床方法及表面肌电仪进行评定,记录静息状态和踝背伸状态胫前肌和腓肠肌的积分肌电值(iEMG),并计算拮抗肌的协同收缩率。结果静息状态iEMG各组之间比较无统计学差异,治疗6周后,BWSTT组和肌电组脑卒中患者下肢Fugl-Meyer评分(下肢部分)、踝关节活动度及踝背伸胫前肌iEMG均提高,协同收缩率数值降低(P<);使用肌电生物反馈、BWSTT对Fugl-Meyer评分(下肢部分)、踝关节活动度及iEMG数值存在交互作用,提示肌电生物反馈与BWSTT联合干预对促进脑卒中患者的康复效果更佳。结论肌电生物反馈和BWSTT组均可使脑卒中患者增强肌力,改善协同收缩率,二者联合可发挥协同作用,增强疗效。表面肌电在脑卒中运动功能评定中有潜在的应用价值。

【Abstract】 Objective To investigate the effect of eletromyographic
biofeedback bined with body weight support treadmill training (BWSTT) on motor function and integrated electromyography (iEMG) of the lower limbs. Method 72 hemiplegic patients were randomized by factorial design method into 4 groups (n=18 each): normal regulation group, BWSTT group, eletromyographic biofeedback therapy group(electromyogram group)and eletromyographic biofeedback bined with BWSTT group (combination group). Patients in each were assessed by Fugl-Meyer scale, range of motion (ROM) of ankle dorsiflexion and iEMG values before and after the treatment. Multifactor ANOVA by Factorial design was conducted in the statistical analysis. Result Before the treatment, there were no statistical significance in Fugl-Meyer scores and iEMG values in the (P > ) 4 groups.
