摘要:目的评价放射性核素201 Tl显像在协助诊断与鉴别诊断乳腺包块中的临床应用. 方法对31例经病理证实的乳腺包块患者均行201 Tl显像(74~111MBq),同时与病理结果进行对比. 结果 11例乳腺恶性包块中,201 Tl检测出10例(%),,,17例无201 Tl摄取(%),3例纤维囊性病变也有201 %. 结论201 Tl显像有望成为临床上广泛应用的一种有效、简便的鉴别诊断乳腺包块性质的方法.

Abstract:AIM To evaluate clinical efficiency of thallium-201scanning as a potential test in differentiating malignant from benign lesions of the Thirty-one women with breast mass proved by biopsy underwent thalli-um-201scanning of the breast(~111MBq),compared with the biopsy results of its anterior and lateral Of11patients with breast cancers,10cases(%)were detected,the smallest detectable cancer in patient who had re-ceived several cycles of chemotherapy did not show thallium uptake.
There was no thallium accumulation in breast mass of17of20patients with benign lesions(%).Three fibrocystic cases showed relative thallium uptake in their breast accuracy of breast mass diagnosis by thallium scanning %.CONCLUSION Thallium scanning has proved to be an easy and effective test in clinical practice in differentiating malignant from benign lesions of the breast.

目前,采用乳腺红外扫描图诊断乳腺包块已经非常普及并积累了许多经验,%~%;%~%[1] ,尤其是假阴性结果使局部活检和治疗方案延迟,病情得以持续发展, Tcm O4 -,99 Tcm -MDP,Ga-67等放射性药物均能被乳腺癌细胞摄取,但由于许多良性乳腺疾患也能摄取它们,故在临床上的应用受到限制[2,3] .已有报道201 Tl也会被许多恶性肿瘤细胞摄取,这其中也包括乳腺癌细胞[4,5] .因此,我们旨在评价在临床工作中如何利用201 Tl显像来协助诊断与鉴别诊断乳腺包块的病变性质.
1 材料和方法
材料前瞻性地研究了1993-03/1994-12间Kosin大学医学院附属肿瘤医院31例女性乳腺肿块患者,术前10d内进行201 Tl乳腺显像,为避免月经周期的影响,显像应放在患者最近一次月经后5~,分为2组:①乳腺癌组:11例,年龄35~72(平均55)岁;肿瘤小于1cm者1例,1~5cm者6例,大于5cm者4例.②良性病变组:20例,年龄26~69(平均44)岁;肿块1~5cm者17例,-phy DS7型(Sophy Medical公司生产)SP
