【摘要】目的:分析胆脂瘤型中耳炎的螺旋CT表现,探讨螺旋CT在临床的应用价值。方法:经手术病理证实胆脂瘤型中耳炎48例,回顾性分析其螺旋CT表现,并与手术结果对照分析。结果:胆脂瘤型中耳炎的螺旋CT表现为鼓室、鼓窦及乳突区软组织肿块,常伴有骨质破坏,其中包括听小骨、盾板、面神经管、半规管、乙状窦及鼓室盖骨质破坏。结论:螺旋CT对胆脂瘤型中耳炎的准确诊断具有很高的价值,对指导临床治疗有重要意义。

Abstract: Objective To analyse the appearance of cholesteatomtous tympanitis on Spiral CT and clinical application. Methods Spiral CT findings of 48 patients with cholesteatomtous tympanitis proved bysurgery and pathology were analysed. Results Spiral CT findings of cholesteatomtous tympanitis included: soft tissue mass in the tympanium, tympanal sinus and mastoid,destruction of the bone including ossicles chain,secutum,facial nerve canal,semricirular canal,sigmoid sinus plate, tegmenttympanic. Conclusion Spiral CT is extremely accurate in diagnosis of cholesteatomtous tympanitis,and has significant value for directing clinic treatm

胆脂瘤型中耳炎是耳部常见病之一,为慢性中耳乳突炎的一种类型,是在慢性炎症的基础上形成的,严重者可引起颅内、外并发症而危及生命。螺旋CT能显示更细微的解剖结构,能对病变侵犯范围及程度作出精确的判断。本文搜集我院经手术病理证实胆脂瘤型中耳炎48例,探讨螺旋CT对本病的诊断、鉴别诊断及对临床的指导意义。
1 材料和方法
本组48例患者,男33例,女15例,年龄11岁~78岁,平均年龄36岁,病程2 a~40 a。临床表现:耳部流臭脓45例,听力减退42例,头痛10例,面瘫2例,并发脑脓肿2例。45例伴有鼓膜内陷、穿孔或增厚,7例外耳道检查窥视不清鼓膜。
采用美国GE公司ProspeedAII双排螺旋CT机,层厚1 mm,间距1 mm,螺距1,窗宽:3 000
