摘要
目的:根据 2007 年、2008 年新疆新型农牧区合作医疗基金筹资及运行情况,重
新调整新疆新型农牧区合作医疗的起付线、封顶线及住院补偿比,通过以地区为统
筹单位,统一补偿模式,从而实现整合资源,降低管理运行成本,提高工作效率,
发挥基金效益,提高抗风险能力的目的,为实现全疆统一补偿模式提供理论依据。方
法:按照经济发展水平、农牧民年均收入水平以及农牧民人口数量、分布,分别从
南疆、北疆、东疆随机分层各抽取 3 地(州)2007 年、2008 年新型农牧区合作医疗
补偿相关数据,再从抽取地州随机分层抽取 3 县(市)进行实地调研,调整患病率、
就诊率、次均住院费用等指标,通过重新调整新型农牧区合作医疗的起付线、封顶
线及住院补偿比,利用已经建立的公式和相关计算方法,运用软件,对参
合农牧民住院费用进行计算。结果:2007 年、2008 年新疆维吾尔自治区北疆地区、
南疆地区、东疆地区,若将乡级、县级及县级以上医疗机构起付线分别设定为 75 元、
70 元、80 元,200 元、150 元 180 元,470 元、450 元、480 元,封顶线全部设为 20
000 元,其住院补偿比例均为乡级医疗机构为 70%,县级医疗机构为 60%,县以上医
疗机构为 50%,各区在保持现有补偿水平的前提下,能够实现基金收支基本平衡。
结论:新疆新型农牧区合作医疗制度实现以“区域为统筹单位”的经济条件、社会条
件、心理条件已经基本具备,通过测算、检验评价,可以实现以“区域为统筹单位”
的统一住院补偿模式,这不仅能有效提高筹资水平、受益面的公平性,而且可以有
效引导就医流向,提高基金使用率。新疆新型农牧区合作医疗制度实现全疆统一住
院补偿模式的条件尚未完备。

Abstract
Objective: According to the situation about fund-raising and new-style rural coopera
-tive medical care in XinJiang in 2007 and 2008 , readjusting operation new cooperative
medical system treatment pastoral 、the paying line、top line、the pensation,
to realize the area as a whole unit, pensation mode, so as to realize the
integration of resources, reduce the management operation cost, improve work efficiency,
increase efficiency, fund of anti-risk ability. Methods: According to the level of economic
development and the farmer e levels, respectively from the BeiJiang, NanJiang,
DongJiang,random extraction two districts in 2007 new farming and related data, through
the readjustment of the new cooperative medical treatment pay pastoral top line line, and
the pensation has been established, use than formula and related calculation
