What are the Goals of this class?
Iscipline:
PRIMARY FOCUS on 8-D FUNCTION:
– To provide a basic understanding of the 8-D steps.
– To give students an experience in applying the 8-D techniques so that
they may use the concepts in their everyday jobs.
– To stress the importance of identifying the root cause in order to
eliminate that cause and prevent recurrence of the problem.
8-D FORM:
–To, time permitting, allow questions on the implementation of the QSI 8-
D form.

What is 8-D?
Iscipline:
8-D FORM:
A QSI format that is used with the ICAR or IPAR as a means
of keeping records of the 8-D problem solving process.
8-D FUNCTION:
8-D stands for 8 Disciplines and is also called TOPS or Team
Oriented Problem Solving
8-D is a problem solving method that effectively:
• Identifies the problem, its root cause(s), its containment,
and its corrective action(s)
• Ensures the corrective actions are implemented / verified
• Prevents the recurrence of the problem.
