【摘要】目的探讨64排螺旋CT在颧骨复合体骨折中的应用价值。方法对25例颧骨复合体骨折患者行64排螺旋CT扫描,,然后用相关软件MPR、SSD和VR进行处理,用不同的旋转轴观察不同角度骨折的三维图像,并比照平面CT进行分析。结果 64排螺旋CT三维图像很好的再现了颧骨复合体骨折的解剖全貌,清晰、直观地显示了骨折部位、骨折块移位的方向和程度,指导临床选择合适的治疗方法;其中23例行内固定手术治疗,2例非手术治疗,均取得了良好的外形和满意的咬合功能。结论颧骨复合体骨折的64排螺旋CT影像显示清晰,在诊疗中有重要作用。

[Abstract] Objective To evaluate the application of 64-slice spiral CT on the plex A total of patients with plex fracture underwent 64-slice spiral CT with a slice thickness of mm. The 3D reconstruction and multiplanar reconstruction (MPR),surface shaded displ(SSD) and volume rendering(VR) were used for the plex 25 patients were diagnosed clearly by three-dimensional CT. The location and moving condition of the fractures was clearly demonstrated. Among 25 patients, 23 undergoing open reduction and firm fixation, and 2 using non-surgical method, achie
ved satisfactory occlusion and appearance after 64-slice spiral CT findings can clearly display the plex fracture, serve as a primary guide in surgical or non-surgical planning.

由于颧骨复合体骨折的临床表现复杂多样,术前诊断尤其是影像学对具体骨折情况的诊断至关重要,在诊断明确的基础上,选择合理的治疗方法,对该类骨折的治疗效果有很大影响[1~3]。以往一般靠普通X线和CT诊断,近年来随着64排螺旋CT在口腔颌面外科各种复杂骨折的诊断中广泛应用,骨折后具体的解剖变化都能清晰显示,三维重建技术在颧骨复合体骨折的诊断中更显示出它独特的优势。本文对2008年11月—2009年9月
