诊断依据： 主观资料：咳嗽有痰，不能全部自行咳出，为II度黄痰。客观资料：疲乏，呼吸浅快，肺部哮鸣音和干湿罗音。
与气管切开，神经反射减弱，吞咽功能障碍有关。
预期目标 病人痰液变稀，容易咳出，肺部无干湿罗音和哮鸣音。
诊断依据 ：主观资料：气促，痰不易咳出。客观资料：呼吸音减弱，肺部哮鸣音。
与患者疲乏无力，肺部疾患有关。
预期目标 病人将能维持有效的换气量。
诊断依据 ：主观资料：气喘，容易疲劳，动则气急。客观资料：意识改变，嗜睡。客观资料 : 呼吸急促，心率增快，X线有肺部感染表现，血气分析有缺氧和二氧化碳潴留。
与气促导致通气、血流比例失调，痰液不能自行咳出，通气差有关。
预期目标 病人的感染控制，无缺氧和二氧化碳潴留，表现出有效咳嗽和呼吸
诊断依据 主观资料：活动减少，易疲劳。客观资料：呼吸困难，紫绀，神志恍惚。
与鼻饲饮食易低于机体需要量，低氧血症有关。
预期目标 病人活动耐力增加
诊断依据 主观资料：体温波动在37到38度之间。客观资料：.右肺上叶异常密度影，考虑炎症，左肺上叶舌段异常密度影，考虑炎症，双肺下叶纤维条索形成，双肺下叶炎症。血常规：白细胞 *10
