　　(五)学历证书、学位证书原件及复印件;
　　(六)原工作单位或居委会、村委会出具的申请实****人员的品行鉴定;
　　(七)户籍所在地公安机关出具的无违法犯罪记录证明;
　　(八)原工作单位或主管部门出具的申请实****人员辞职或解聘文件;
　　离退休人员申请实****的，须提供离退休证或离退休批准文件原件及复印件;
　　实****前从未参加过工作的，由毕业院校或所在的居委会(村委会)出具相关证明材料。
　　(九)人事档案保管合同复印件、档案托管证明及其简历;
　　申请实****人员的法律职业资格或律师资格档案不在本省的，由省司法厅负责向原主管司法行政机关调取。自行携带档案的，省司法厅不予接受。
　　(十)一寸近期免冠照片一张。

　　拟兼职从事律师执业的人员申请实****时，除提交(一)、(二)、(三)、(四)、(五)、(六)、(七)、(九)、(十)等项材料外，还应提交所在的法学院校(系)、法学研究单位出具的同意其实****的证明文件。
　　第八条申请实****人员和接受申请实****人员的律师事务所应当签订《实****合同》，明确以下事项：
　　(一)申请实****人员姓名;
　　(二)接受申请实****人员实****的律师事务所名称、地址;
　　(三)指导律师的姓名、执业证书号码、执业年限;
　　申请实****人员和接受申请实****人员的律师事务所的权利与义务，以及实****的起止时间;
　　(四)申请实****人员实****期间劳动报酬的约定。
