【摘要】目的:探讨应用DHS治疗股骨粗隆间骨折的适应症、治疗方法和临床疗效。方法:应用DHS治疗股骨粗隆间骨折共157例。结果:157例
中142例得到6个月~7年随访,其中优119例,尚好21例,差2例。结论:应用DHS治疗股骨粗隆间骨折A1、A2型,临床效果可靠,固定牢固,可早期下地活动,患肢功能恢复良好;但不适用于A3型骨折。

【Abstract】Objective:The discussion applies the DHS treatment thighbone thick prosperous bone fracture the indication, the method of treatment and the clinical curative effect. Method: Using DHS treatment thighbone thick prosperous bone fracture altogether 157 :In 157 examples 142 examples obtain 6 months ~7 year to make a follow-up visit, superior 119 examples, still good 21 examples, difference 2 :The conclusion applies DHS treatment thighbone thick prosperous bone fracture A1, A2, the clinical effect reliable, fixed is reliable, but under the early time the activity, contracts the extremity function to restore good; But ill uses in the A3 bone fracture.
【Key words】Thighbone thick prosperous bone fracture; Treatment; DHS股骨粗隆间骨折(Interochantericfraeture)是老年人常见损伤,偶见于青壮年,临床上多主张手术治疗[1],我们于2000年11月~2007年5月应用DHS(动力髋螺钉)治疗股骨粗隆间骨折157例,临床效果满意,现报告如下:
1临床资料
:本组157例患者中男性73例,女性84例,年龄21~95岁,,受伤部位:左侧96例,右侧61例。致伤原因:跌伤110例,交通伤38例,坠落伤9例。按AO的müller分类[2]:A1型:63例,A2型:92例,A3型2例。
:病人多表现为伤后即感髋部疼痛,活动受限,不能站立或行走,下肢短缩及外旋畸形明显,无移位者上述症状可轻微,查体可见患侧大粗隆升高,局部肿胀及瘀斑,深压痛,纵向叩击痛常呈阳强性及强阳性。本组病历均经X线拍片及CT确诊。
