【摘要】目的比较研究腹腔镜阑尾切除术和开腹阑尾切除术后血液凝固状态的改变。方法检测39例开腹阑尾切除术和16例腹腔镜阑尾切除术患者术后24h血液凝血酶原时间(PT)、部分凝血活酶时间(APTT)、D-二聚体(D-D)、纤维蛋白原(FIB)。两组入选标准分别为急性期的阑尾炎,既往无导致血栓形成的高危因素。开腹组手术时间为40~92分钟,平均(±)分钟,腹腔镜组手术时间为45~98分钟,平均(±)分钟,差异无统计学意义(P>)。。结果术后24h两组PT 、APTT :A均值在正常范围,组间比较差异无统计学意义(P>);而D-D、FIB均值均高于正常范围,组间比较差异无统计学意义(P>)。结论腹腔镜阑尾切除术后能引起血液的高凝状态。但与传统的开腹手术比较,风险没有增加。

【Abstract】 Objective To study the changes of homeostasis after mon bile duct exploration versus mon bile duct exploration. Methods Thirty-nine patients with mon bile duct exploration and sixteen patients with la
mon bile duct exploration were enrolled in this study. The homeostasis parameters including PT, APTT, D-D, FIB, were measured at 24 hours after the operation. Mean operaton time was ± minutes (range, 40~92minutes) in open group, and ± minutes (range, 45~98 minutes) in laparoscopic group. Statistical analysis was done with ’s two-sided t-test for paired samples. Results The mean values of PT, APTT:A were normal and mean values of D-D, FIB,were significantly increased in both mon bile duct exploration and mon bile duct exploration.
